The application forms can be found on the Board’s website found here: https://www.pisb.nd.gov/licensing/application-forms. Those forms went “live” on the Board’s website today.

Revised and New Application Forms
Thursday, June 29, 2023
The Board has revised application form pages 1, 2A, 4, and 6. The revised forms are also available on the Board’s website. The Board will continue to accept the "old" forms until July 31, 2023, after that date if old forms are submitted they will be returned to the agency and/or applicant as incomplete.
Minor revisions occurred to application form pages 2A, 4, and 6. A number of revisions occurred to application form page 1:
- Removal of “appeal” under status of application
- Addition of question 2A.
- Changes to questions 11, 14, 16, 23
- Addition of question 23A
- Removal of notary requirement
- Changes to fees and the addition of the fee for the Armed First Responder in School or Ambulance or Firefighter Crew Training Program Course Certification.
The Board also developed a new application form, page 7. This application form is specifically related to the Armed First Responder in School or Ambulance or Firefighter Crew Training Program Course Certification.
The revisions and new form were implemented based on feedback from the industry, to (hopefully) alleviate some common issues the Board sees on submitted applications, due to changes to fees, and to allow applications for the Armed First Responder in School or Ambulance or Firefighter Crew Training Program Course Certification to be submitted.
